Why the governance layer is missing

Control system vendors (GE Vernova, Siemens, Hitachi, Schneider, Oracle) build operational performance — monitoring, control, optimisation. Gap 1 — The Override Gap

Operators override AI for legitimate reasons — local knowledge, asset conditions, safety judgement. Without a governance record, that sound human judgement is indistinguishable from an undisciplined deviation. NERC CIP enforcement — 2024

50+ enforcement actions in 2024 under NERC CIP (the US grid reliability standard) for change management, access control, and audit trail gaps in utility operations.
